Reading about the recent Boeing fuselage failure, I see that the fixings that apparently failed, were found to be in need of:
"additional tightening"​
Wondering if you've come across other euphanisims for things that have not been done quite right?

I noticed and thought that too, and other aircraft have been found with the same fault.

The thing that I don't get is why the hole was there in the fuselage? The hole was plugged with a plug that was not fixed properly. Just don't make the hole.
 
My understanding is that the ‘hole’ is there for configurations of the aircraft where it is operated with a greater seating density requiring additional escape capacity for compliance. Still needs the bolts to be tight though, so we’re they not tight enough from the get go or had they loosened over time?

Some build versions have that door, to meet the 90 seconds to evacuate the plane. Depends on airline usage patterns, short/long haul, evidently.
Some aircraft versions, with a real door, rather than a plug, should be OK(ish). Worrying though.

youd think that there’s an exact torque setting for some thing this critical, let’s face it, if an A2 rear shock top mounts have torque and stretch bolts, which is hardly a critical area, surely this is very odd indeed?
 
The door is a real emergency exit door. The only difference in this configuration is the interior panelling covers all bar the window. The exterior release handle is still there and functional. BTW there is also a corresponding door on the other side of the fuselage.

Once had to read a CIA report on Iraq's UAV program, which ended in 2003 due to 'rapidly changing political circumstances'.
