Looks like water got under and rust broke the bond. Friction then ripped apparat the lining material.
What brand were they?

That looks terrible and extremely dangerous. Hope this was observed during routine maintenance and not whilst nipping about; could have been an unthinkable outcome with brakes locking up at speed.

Happen to me as well. Typically if car has been standing out in the rain for a long time and discs starts to rust against the pads.

You would know when it happens as the you would need to pump the brake pedal to compensate the sudden longer travel of the calipper piston. Also the brakes will rumble as metal against metal.
 
Ok. long story short.
These discs came from my lads A2.
These discs came from my lads A2 on Friday just gone, he was driving back from Minehead returning from his holiday at Butlins with his young family and had to pull into Gloucester services north bound for toilet duties.
As he was slowing down on the slip road heading for the parking area he heard a bang and the brake peddle seemed to drop slightly under his foot.
He could then hear a crunching kind of noise as the wheels turned and eventually found a parking bay.

At last years MOT 16/09/22, his discs were an advisory stating "Front discs worn, pitted or scored but not seriously weakened, Both sides, Now I hold my hands up here, I should have replaced them but he didn't have the money at the time and said we could do them at a later date and then we simply forgot .
Now move on a year and his car went in for its MOT. This was done at the garage where I always have my MOTs done and used them for years with no problems but this year the young apprentice got his cert to do MOTs and he did my lads.
Result, It passed with no advisories.
luckily, he is an AA member and the guy turned up around two hours later and said it needed new discs and pads and drove off . Within an hour he returned with a set of PAGID discs and new pads and had them fitted within an hour leaving the old stuff with the lad. This cost him £185.

Now im not going to go to my MOT place all guns blazing and complain that this wasn't picked up during the MOT, I might just have a quite word with the young lad and show him the photos.
What im trying to say here is, check your discs lads.
I guess im going to get some stick here for not changing them last year and i would understand.
 
Replaced my discs and pads to Brembos after that same advisory last year. The challenge I face now is not using the car enough. I must take it out on a regular basis, run it up to temp, and use the brakes. Glad everyone was okay.

I’m bot sure how your MOT tester could have picked that issue up. From his view point everything would look fine. He would have had to strip the brakes and unless the lining materials were moving he still may not spot the problem.
 
Had to replace rear shoes in wife's A2 as the lower part of the lining had delaminated after it stood unused quite a while.
Younger son reported a strange noise from the front brakes on his Hyundai which he only uses occasionally as working from home quite a lot of the time. One of the linings had parted company completely.
Just replaced the handrake shoes on my daily driver after the linings had started to seperate from the backing.
Mixture of Ate, Drivetec and Bosch brands all failing within last year.
Now only buying Ferodo from local motor factors as a bit suspicious of some mail order parts, just have to see how it goes.
 
I dont think that MOT tester can indicate cracked pads, if bit is missing...maybe but way pads are located its hard to spot more than wear indication, had this happening on cars with one pad being tottaly gone...car wil stop...with noticable noise ...and sparks possibly
 
This is not that apprentice fail whatsoever. He saw rusty brakes which still worked during balance brake testing and were perfectly intact. The pads broke on the way back so I would spare that poor apprentice any horror photos because honestly he did the job properly. They can't take apart entire car and xray all the parts to find cracks in worn out parts. It's our job to make sure when they are on advisory to replace them asap.
 
I’ve seen this on a Merc with low mileage, all genuine original parts. The thought was as above that the pads had stuck to rusty discs. Not something you’d ever pick up on a Mot.

Ultimately service your vehicle correctly and don't look to blame others when things go wrong, advising them again would probably have not made you change them anyway.
 
Have to agree with the above - I don’t reasonably see what else all parties could have done other than change the parts earlier - but then if the disks appear serviceable I wouldn’t be in a rush to change them.

What I will say is that once a year, now being a good time before it gets too cold, make it a routine to rotate your tyres around to even out wear - this habit will allow you to inspect the brakes more closely.
